5)b) The door chime in portable 7 was not working so there was no sound when the exit door was opened to alert staff and prevent children from leaving unsupervised. 5)c)vii) During outdoor play, two staff were engaged in untying a knot in a child's shoe while responsible for active supervision of many other children. The staff were not actively supervising the children in care.
